Community Health Services, Inc. is seeking a Registered Dietician in our Adult Medicine clinic. The Registered Dietician delivers superior nutritional consultation and education services while supporting population health efforts by adhering to clinical quality measures and using data to monitor, identify, and address variances in outcomes across the patient panel.

The successful candidate will:

  • Possess a bachelor’s degree in nutrition or a related field
  • Have one (1) year of experience proving nutrition services and/or diabetes education in a clinical setting
  • Be a certified Diabetes Educator
  • Possess a valid State of Connecticut license as a dietician/nutritionist
  • Be certified by the Academy of Nutrition and Dietetics
  • Possess a CPR certification at the healthcare provider level or eligible to obtain certification within 90 days of hire
  • Have CPT-4 and ICT-10 coding experience
  • Provide nutrition care services to an active panel of patients, based on assessments, in accordance with professional standards of care as well as organization policies, and procedures, and protocols and department and organizational scheduling protocols
  • Perform individualized nutrition assessments of dietary history and food habits to provide comprehensive nutrition counseling and education
  • Review and evaluate medical and dietary history, laboratory data, and physical assessments, where necessary, to evaluate patients for nutrition-related risks
  • Evaluate psychosocial, function, and behavioral factors related to food access, selection, and preparation as well as physical activity
  • Provide nutrition education that respects the patient’s health literacy and social determinants of health needs
  • Provide specialized nutrition care and support to chronically, critically, and acutely ill populations and educate patients as appropriate
  • Assist patients in identifying self-management goals and document progress or outcomes as appropriate
  • Act as a nutrition consultant to the organization and collaborate with primary care providers as needed or requested to ensure patients’ nutrition needs are met
  • Prepare and/or maintain instructional and information materials for patients regarding nutrition and dietary guidance
  • Support population health efforts by using data analytic tools to monitor and continuously improve outcomes across various patient populations
  • Submit accurate, timely documentation that captures plan of care and clinical decision-making that complies with federal, state, third-party payer, and organizational documentation, coding, and billing standards
  • Adhere to clinical and administrative quality measures as demonstrated by ongoing and focused professional practice evaluation
  • Participate in department initiatives designed to improve the efficient and effective function of clinical operations
  • Contribute to systems and process designed to ensure the quality and safety of clinical care by participating in ongoing and focused professional practice evaluation and peer review activities
