Community Health Services participates in the NHSC Loan Repayment Program

Since 1970, Community Health Services has existed to provide comprehensive health care services to the residents of Hartford and Windsor. The Director of Infectious Diseases will provide clinical and operational oversight of the Ryan White program and consulting services to the organization regarding the care and treatment to patients with, or at risk of contracting, infectious diseases.

The successful candidate will:

  • Be a graduate of an accredited medical and an accredited residency program in a relevant clinical discipline
  • Be board certified in Infectious Disease
  • Possess a valid State of Connecticut license as a physician
  • Have at least two (2) years of successful experience as a practicing infectious disease physician and at least one (1) year of clinical administrative management experience.
  • Have an active CPR certification at the healthcare provider level or be eligible to obtain certification within 90 days of hire
  • Have experience with CPT-4 and ICT-10 coding

Clinical Leadership

  • Provide direct and indirect clinical leadership to the Ryan White and infectious disease program, including recruitment, orientation, professional development, clinical supervision, evaluation, quality oversight
  • Act as a subject matter expert for the organization on matters related to infectious diseases
  • In collaboration with the Clinical Director, ensure implementation of ongoing and focused professional practice evaluation and metric-driven provider evaluation; support peer review processes as needed to ensure appropriate levels of quality, safety, and citizenship and ensure provider performance issues are addressed fairly and in a timely manner
  • Participate and collaborate with the Clinical Director on population health initiatives that improve patient care, promote provider and patient engagement, and lower the cost of care while improving outcomes
  • Act as an exemplar clinician by maintaining an active caseload of infectious disease and internal medicine patients in accordance with organization policies, and procedures, and protocols;
  • Participate in clinical call rotations and act as a backup resource for providers during after-hours coverage or other emergency situations
  • Supervise advanced practitioners via formal collaborative and supervisory agreements as well as through informal means, as necessary
  • Develop and maintain relationships with infectious disease clinical partners and potential clinical partners to foster linkages with specialists and sub-specialists who may be able to complement the infectious disease services provided at the health center
  • Actively review and remain well-informed of evidence-based practice standards, health care advances and incorporate such standards and advances into the program’s clinical practices
  • Support the professional development of Ryan White and infectious disease staff and maintenance of clinical competencies by identifying training needs and promoting ongoing clinical education opportunities
  • Ensure program staff compliance with the federal, state, local, and organizational documentation, coding, and billing standards

Administrative Leadership

  • In collaboration with the Clinical Director, support the successful implementation of the department service line business plan
  • Ensure the efficient and effective function of Ryan White program clinical operations, including patient scheduling, staffing of clinical and support staff, space management, and technology, by participating in the identification and resolution of day-to-day operational issues
  • Use data analytic tools to develop, establish, and continuously improve systems to monitor and improve clinical and operational outcomes in the Ryan White program
  • In collaboration with the risk managers as necessary, oversee the appropriate resolution of patient grievances regarding the quality of care
  • Contribute to the growth and improvement of the clinical service line by identifying new clinical services and/or improvements to existing services
  • With Human Resources, manager recruitment, onboarding, retention, and engagement of Ryan White and infectious disease program staff
  • Provide clinical input on organization committees, as needed, including the Quality Assessment and Performance Improvement Committee and Infection Control Committee

Community

  • Develop and sustain positive and productive relationships with community infectious disease leaders to advance organizational objectives
  • Maintain a positive image of the health center in the community by representing the organization on boards, commissions, councils, workgroups, and committees as well as at government and community events
  • Perform all duties in accordance with CHS Core Values, Compliance Program, and other policies and procedures
  • Participate in quality and process improvement efforts in the department and the organization as requested or as directed
  • Complete training and professional development requirements in a timely manner
