Community Health Services, Inc. is seeking a Care Coordinator  for our Adult Medicine Department. The incumbent’s primary responsibility is to consistently provide patient-centered administrative support to our Nurse Care Coordinator Supervisor in patient care coordination processes. Different tasks included in the nursing assistant role are related to home care orders, prior authorizations, scheduling hospital discharge follow up appointments and developing and implementing Population Health Management Initiatives. Position reports directly to the Adult Medicine Nurse Care Coordinator Supervisor. This multi-disciplinary work will be in alignment with Community Health Services’ Organizational Core Values and the Patient Centered Medical Home approach to care.

The successful candidate will:

  • High school diploma or equivalent

  • CPR certification at the healthcare provider level or eligible to obtain certification within 90 days of hire

  • Proficiency in the use of Microsoft Office applications, including Word, Excel, and Outlook

  • Demonstrate superior customer service skills

  • Be able to produce high quality work efficiently and under tight deadlines in an environment with changing priorities

  • Have proven success handling confidential information

  • Possess the ability to build and maintain successful working relationships with internal clinical and administrative staff as well as external stakeholders, including specialists who treat CHS patients and their office staff

  • Be comfortable with or be able quickly to learn electronic practice management systems

  • Have the willingness and ability to work as part of a team and to demonstrate the highest standards of professionalism

  • Candidates who are bilingual in English and Spanish with prior experience working in a multiple provider medical practice or with diverse socio-economically disadvantaged populations are preferred.
