Welcome to Community Health Services
We are pleased to inform you that we are resuming in-office visit for routine care. We have instituted every precaution to make CHS a COVID-19 safe environment. For patients that are at high-risk or are not comfortable coming to the office at this time, we continue to offer Telehealth visits.

Please note the following procedures.
Safety continues to be our top priority.
Use hand sanitizer and wash your hands often.
Use masks if respiratory symptoms are present for staff or patients.
Triage patients and use a negative pressure room as indicated.
Monitor staff and patients for infectious/communicable diseases.
Conduct testing and screening as indicated.
Keep our environment clean and equipment disinfected as needed.
We protect the health of our patients and staff.
We provide and administer vaccinations.
